چکیده

Background context Correction of adult spinal deformity (ASD) by long segment instrumented fusion is an increasingly common surgical intervention. However, it associated with high rates complications and revision surgery, especially in the elderly patient population. The construct stiffness thoracolumbar has been postulated to lead a higher incidence proximal junctional kyphosis (PJK) failure (PJF). Several cadaveric biomechanical studies have reported on techniques reduce PJF/PJK. As yet, no overview made these studies. Purpose To summarize evidence all that assessed PJK/PJF following ASD Study design A systematic review. Methods EMBASE MEDLINE databases were searched for human animal investigating effect various Studied techniques, test methods, range motion (ROM), intervertebral disc pressure (IDP) other relevant outcome parameters documented. Results Twelve met inclusion criteria. Four included non-human material. One study investigated prophylactic application cement augmentation (vertebroplasty), whereas remaining semi-rigid fixation achieve gradual transition zone forces at end construct, so-called topping-off. An increased terms ROM compared pedicle screw constructs was demonstrated sublaminar tethers, tape, pretensioned suture loops, transverse hooks laminar hooks. Furthermore, reduced IDP found after tapes Finally, two-level vertebroplasty resulted lower vertebral compression fractures flexion-compression experiment. Conclusions variety involving either posterior or reinforcement bodies, biomechanically assessed. low number variation protocols hampers direct comparison different techniques. determination what constitutes optimal its translation clinical practice, would aid further development Even though biomechanics are extremely important PJK/PJF, patient-specific factors should always be taken into account case-by-case basis when considering apply technique.

Proximal Junctional Kyphosis and Proximal Junctional Failure Following Adult Spinal Deformity Surgery

The purpose of this review is the current understanding of proximal junctional kyphosis (PJK) and proximal junctional failure (PJF) following adult spinal deformity (ASD) surgery. We carried out a systematic search of PubMed for literatures published up to September 2017 with "proximal junctional kyphosis," "proximal junctional failure," and "adult spinal deformity" as search terms. A total of ...

Proximal junctional kyphosis in adolescent idiopathic scoliosis following segmental posterior spinal instrumentation and fusion minimum 2 years follow-up

Abstract   Background: To evaluate proximal junctional segment changes in Adolescent Idiopathic   Scoliosis(AIS) the posterior spinal fusion and also instrumentation also and   finding of probable risk factors, were all considered in this study.   Methods: We retrospectively reviewed radiographs of 121 consecutive patients   who underwent posterior spinal fusion for AIS from T3 or below, with...

Diagnosis and Classification of Proximal Junctional Kyphosis and Proximal Junctional Failure

Pedicle screw instrumentation constructs have become a cornerstone in the treatment of adult spinal deformity and instability. They are known to provide greater rigidity and enhanced ability to correct and maintain spinal alignment. Biomechanical data, however, demonstrate that increased construct stiffness is associated with increased loading within adjacent segments [1– 9]. Increasingly stiff...

Proximal Junctional Kyphosis in Early Onset Scoliosis

In adults and adolescents, proximal junctional failures above deformity constructs have been well established with different risk factors identified; however, the incidence, nature and clinical significance of this kyphosis remains unclear. Several reports in the literature have shed light on PJK in adults and adolescents with variable definition, incidence, prevalence, acuteness, risk factors,...

Prevalence and Consequences of the Proximal Junctional Kyphosis After Spinal Deformity Surgery

The aim of this study was to estimate the prevalence and patient outcomes of proximal junctional kyphosis (PJK) in pediatric patients and adolescents who received surgical interventions for the treatment of a spinal deformity.Literature was searched in electronic databases, and studies were selected by following précised eligibility criteria. Percent prevalence values of the PJK in individual s...
